Ladies and Gentlemen:

The undersigned understands that JEDEC Committee membership is limited to companies and independent entities of companies that (1) manufacture solid state products, or provide related services or equipment, and (2) participate in the United States market.

We agree to participate in the activities of the JEDEC Engineering Committee(s) listed below and to pay an annual fee in accordance with the following schedule:

Participation in one committee and its subcommittees: $3,000.00

Participation in two committees and their subcommittees: $4,000.00

Unlimited Committee and Subcommittee Participation: $6,000.00 MAX

I understand that this fee is payable upon return of this Application form and whether I attend the Committee meetings and regardless of the number of meetings scheduled during each calendar year. I also understand that I will be invoiced again at the beginning of the calendar year.
